Marcel Siem beat Tom McKibbin in a play-off to claim the Italian Open title on Sunday and claim his sixth European Tour victory in just his fourth tournament since returning from hip surgery in February.
Sim and McKibbin both finished at 10 under, and Sim sealed the win with a birdie on the first playoff hole.
Sim led club leader McKibbin by three shots through eight holes of the final round, but was one shot behind going into the 18th hole because of four bogeys.
But Sim holed a 22-foot putt to finish at even-par 71 and force a playoff.
“Holting the putt on the 18th hole was one of the coolest moments I’ve ever had playing golf, and to do it again in a playoff was awesome,” Sim said.
McKibbin shot a bogey-free 65 in his final round at Adriatic Golf Club in Cervia.
McKibbin missed out on a second DP World Tour title by one stroke along with American Sean Crocker, with McKibbin getting the consolation prize of playing in the British Open, while Crocker finished tied for third with Jannik de Bruyn.
Sim, 43, won his first European Tour title more than 20 years ago. He lost his tour card in 2021 and regained it in qualifying last year. He then won the Indian Open early last year, his first tour title in more than eight years.
